Chronic Care Management

Living with chronic medical conditions can be emotionally and financially stressful. Kennedy Health Alliance can take the hassle out of your healthcare with Chronic Care Management (CCM). 

CCM utilizes a specially trained Registered Nurse who acts as your personal healthcare coordinator to navigate your individual healthcare needs, resulting in overall health improvement and potential cost savings.

As a participant, you'll have convenient access to a personal healthcare coordinator who will work closely with your doctor to:

  • Create a personalized plan to address your individual healthcare needs
  • Regularly assess your healthcare plan and notify you of necessary changes
  • Remind you when you're due for preventative care services
  • Review your medication regimen, and help find discounted medication(s), if needed
  • Coordinate your care among various doctors, health providers and community facilities
  • Help you prevent costly hospital re-admissions by ensuring you understand and can follow your care plan.

CCM services are available to Medicare patients who suffer from two or more chronic medical conditions, including (but not limited to):

  • Diabetes
  • Cancer
  • High Blood Pressure
  • High Cholesterol
  • Heart Disease
